Under brand names like Ozempic and Wegovy, this once-weekly injection has transformed what is possible in medically supervised weight loss. Semaglutide belongs to a class of drugs called GLP-1 receptor agonists. GLP-1 stands for glucagon-like peptide-1, a hormone that your gut releases naturally after you eat. It has several important effects: it signals your pancreas to produce insulin in response to rising blood sugar, it suppresses glucagon (which would otherwise raise blood sugar), and it slows the rate at which food leaves your stomach (gastric emptying). Most importantly for weight loss, GLP-1 acts on receptors in the hypothalamus and brainstem — areas of the brain that regulate hunger and satiety — reducing appetite and increasing the feeling of fullness.

What the Clinical Trials Show

Who Qualifies for Semaglutide

Wegovy (semaglutide 2.4 mg) is FDA-approved for adults with a body mass index (BMI) of 30 or higher, or a BMI of 27 or higher in the presence of at least one weight-related health condition such as type 2 diabetes, high blood pressure, high cholesterol, or obstructive sleep apnea. Ozempic (semaglutide 0.5 mg to 2 mg) is FDA-approved for type 2 diabetes management, though it is widely used off-label for weight loss.

Semaglutide is not appropriate for everyone. It should be avoided in patients with a personal or family history of medullary thyroid carcinoma or multiple endocrine neoplasia syndrome type 2. It is not recommended during pregnancy. Patients with a history of pancreatitis or severe gastrointestinal disorders should discuss risks carefully with their physician.

Expected Weight Loss

Results vary by individual, but most patients begin to notice appetite reduction within the first two to four weeks. Meaningful weight loss — five percent or more of body weight — typically becomes apparent between weeks eight and sixteen. The greatest rate of weight loss generally occurs in the first six months, with continued but slower progress through twelve to eighteen months. Many patients achieve fifteen to twenty percent total body weight loss with appropriate dosing and lifestyle support.
